Sample Submission Forms

Samples must be delivered to the laboratory between 0900 and 1700 Monday to Friday in both Sydney and Melbourne.

Always include a completed copy of the Sample Submission Form with the parcel (see forms below) or registration may be delayed.

Incoming samples are received by the Sample Reception team who open the packages. As part of our QA System, several checks are made before the project is registered:

• Receipt temperature (for perishable samples only)
• Sample integrity (visual check)
• Number/type of samples correlates to information on the submission form
• Labelling on physical sample(s) correlates with sample information on submission form
