Sample Submission Forms
Samples must be delivered to the laboratory between 0900hrs and 1700hrs, Mondays to Fridays. For all samples send to Eurofins | ams for analysis or testing, please include a completed copy of the Sample Submission Form. The sample submission form can be downloaded from the links shown below.

When sending samples to the lab for analysis, please take note of the following.
- Download & Complete the respective Sample Submission Form (Please include submission form in or on the box of samples).
- Ship samples and submission forms to the following:

Incoming samples are received by the Office Administrator who opens the packages. They will check the integrity of the samples and that the number of samples received correlates with what is indicated on the Sample Submission Form (SSF).
